Overview
Resveratrol is a naturally occurring polyphenolic compound predominantly found in the skin of red grapes, berries, and peanuts. It is recognized for its antioxidant, anti-inflammatory, and potential anticancer properties. Resveratrol exerts its biological effects primarily through modulation of various signaling pathways, including the sirtuin 1 (SIRT1) pathway, which is implicated in cellular stress resistance and longevity. Its ability to activate the SIRT1 pathway links resveratrol to mechanisms of aging and metabolic regulation, making it a subject of interest in the study of age-related diseases and conditions such as type 2 diabetes and cardiovascular diseases.

Recent investigations into resveratrol have focused extensively on overcoming its poor aqueous solubility and low oral bioavailability through advanced formulation technologies. Researchers have developed multiple delivery systems including spray-dried inhalable powders, polymeric nanoparticles, liposomes, microneedles, and polymer-based emulsions, each designed to enhance bioavailability and enable targeted tissue distribution. A spray-dried resveratrol formulation for pulmonary delivery achieved a lung-to-plasma area under the curve ratio of 282 compared to 77 for micronized resveratrol, demonstrating superior pulmonary accumulation with excellent tolerability. Hydrophobic resveratrol derivatives showed reduced phase II metabolism and improved permeability across intestinal models, while carrier systems based on selenium nanoparticles, alginate matrices, and mesoporous silica nanoparticles have been engineered to enhance both stability and controlled release.
Recent studies have demonstrated resveratrol's therapeutic potential across multiple disease contexts through distinct mechanistic pathways. In neurological disease models, resveratrol preserved blood–brain barrier integrity and reduced infarct area in stroke by modulating the sFRP4/Wnt signaling pathway and suppressing inflammatory mediators. The cis-isomer of resveratrol inhibited mitochondrial endonuclease G activity and improved motor symptoms in a mouse model of Parkinson's disease, whereas the trans-isomer enhanced endonuclease activity—revealing unexpected opposing effects of resveratrol isomers on EndoG. In a neurogenic bladder model, resveratrol suppressed the AKT/mTOR/S6K signaling pathway, reducing bladder fibrosis, alleviating urothelial pyroptosis, and improving urodynamic function.
Resveratrol has emerged as a potent inducer of ferroptosis in pancreatic cancer models, downregulating ferroptosis suppressors including GPX4 and CTSB while activating the HIF-1 and transforming growth factor-β pathways. In hepatocellular and colorectal cancer contexts, resveratrol-loaded nanocarrier systems achieved enhanced tumor accumulation and cytotoxicity through both direct antiproliferative effects and immune modulation, with resveratrol prodrug nanoparticles triggering immunogenic cell death and activating natural killer and NKT cell responses. For inflammatory and oxidative stress-associated conditions, resveratrol delivered via selenium nanoparticles attenuated silicosis-induced pulmonary inflammation and fibrosis by restoring antioxidant enzyme activity and suppressing NF-κB and transforming growth factor-β-mediated fibrotic signaling. Similarly, intra-articular resveratrol formulations reduced swelling and inflammatory cytokine production in gouty arthritis and osteoarthritis models while preserving cartilage architecture and suppressing macrophage-mediated inflammation.
Recent advances in formulation science have extended resveratrol applications to previously challenging anatomical sites and disease contexts. Three-dimensional-printed microneedles and mucoadhesive nanofibrous systems have enabled intradermal and nasal delivery of resveratrol-loaded nanoparticles for topical oxidative stress and inflammatory disorders, while gelatin-stabilized high internal phase emulsions demonstrated superior thermal and ultraviolet protection with controlled intestinal release kinetics. Cyclodextrin-based polymers enhanced resveratrol encapsulation and intrinsic antioxidant activity for cartilage repair applications, improving human chondrocyte metabolic activity and extracellular matrix deposition. The synthesis of resveratrol dimers through biomimetic radical coupling has generated dihydrobenzofuran-containing scaffolds including δ-viniferin and gnetin C, opening new structural possibilities for therapeutic development.
