Overview

Chronic renal insufficiency is a clinical term used for long-standing, progressive loss of kidney function. In contemporary biomedical literature, it is often used interchangeably with chronic kidney disease (CKD) or chronic renal failure, although CKD is the preferred modern term. The condition is characterized by persistent structural or functional kidney impairment, reduced glomerular filtration rate, albuminuria, and progressive tubulointerstitial fibrosis. Over time, chronic renal insufficiency can lead to complications such as anemia, hypertension, cardiovascular disease, mineral and bone disorders, metabolic dysregulation, and eventual end-stage kidney failure.

Biologically, chronic renal insufficiency reflects a final common pathway of diverse kidney injuries, including diabetic nephropathy, ischemic injury, immune-mediated disease, and age-related decline. Recent research emphasizes mechanisms such as transforming growth factor-beta signaling, MAPK pathways, oxidative stress, mitochondrial dysfunction, cellular senescence, podocyte injury, and renal fibrosis. Because it is tightly linked to type 2 diabetes, heart failure, cardiovascular disease, and systemic inflammation, chronic renal insufficiency is also a major target for cardio-kidney-metabolic therapies, including sodium-glucose cotransporter-2 (SGLT2) inhibitors, finerenone, and prolyl hydroxylase inhibitors.

Focus of Latest Publications

Recent studies have treated chronic renal insufficiency primarily as a progressive disease state, a risk modifier for other clinical outcomes, and a therapeutic target. Several papers focused on CKD progression and fibrosis, describing chronic kidney disease as a condition driven by renal fibrosis, podocyte injury, mitochondrial damage, and tubulointerstitial remodeling. One medicinal chemistry study designed potent, selective, peripherally acting JNK3 inhibitors for chronic kidney disease, based on the concept that CKD is characterized by progressive fibrosis and podocyte injury driven in part by TGF-β-dependent MAPK signaling. Another multi-omics study reported that burdock seed aglycone alleviated renal fibrosis by restoring mitochondrial oxidative phosphorylation function, reinforcing the central role of mitochondrial dysfunction in chronic renal insufficiency. Similarly, studies on DYRK1A/FOXO1 signaling, CaMKIIδ expression, and mitophagy-related pathways linked cellular senescence, lipid accumulation, and fibrosis severity to CKD pathology.

A substantial portion of the recent literature examined chronic renal insufficiency in the context of diabetes and cardio-renal risk. SGLT2 inhibitors were evaluated both for prevention of incident CKD in type 2 diabetes and as cornerstone therapies recommended for patients with CKD, heart failure, or atherosclerotic cardiovascular disease. finerenone was also highlighted as a renoprotective therapy, with studies assessing its effects in CKD associated with type 2 diabetes, including patients with a history of nephrectomy and those with different pre-treatment eGFR trajectories. A separate trial program, TRANSCEND-CKD, was designed to test retatrutide in patients with chronic kidney disease, reflecting interest in metabolic therapies that may influence kidney disease pathways. In type 1 diabetes, finerenone was specifically studied for efficacy and safety in persons with CKD, underscoring the expansion of mineralocorticoid receptor antagonism into broader CKD populations.

Other studies addressed complications and prognostic consequences of chronic renal insufficiency. CKD was associated with higher perioperative morbidity in surgical settings, including postoperative acute kidney injury in patients with pre-existing CKD and adverse perioperative outcomes after TURBT. In transplant-related and post-acute injury settings, CKD was described as a downstream consequence of acute kidney injury, hematopoietic stem cell transplantation conditioning, or liver transplantation. Chronic renal insufficiency also appeared in studies of cardiovascular and hematologic risk, including platelet activation and thrombosis, fracture risk, mortality risk, and susceptibility to cardiovascular complications in patients receiving renal anemia therapies such as roxadustat and daprodustat. In addition, CKD was linked to systemic inflammation, fatigue, sarcopenia-related pathways, and chronic pain syndromes such as fibromyalgia in hemodialysis patients.

Several publications focused on biomarkers, prediction, and staging. Renalase was investigated as a peptide biomarker in chronic kidney disease, with emphasis on assay-dependent variability in quantification. KIM-1 was used in a serum-based early warning approach combining surface-enhanced Raman spectroscopy and artificial intelligence for early diagnosis of CKD. Other work explored urinary fluorogenic reporters for noninvasive detection and staging of kidney fibrosis, targeted LC-MS/MS health surveillance panels, and machine learning models for forecasting incident CKD and postoperative AKI in CKD patients. These studies collectively suggest a strong research trend toward earlier detection, risk stratification, and individualized prediction of chronic renal insufficiency progression.

Chronic renal insufficiency was also studied in relation to systemic comorbidity and population health. obesity-related phenotypes, body roundness index, triglyceride-to-HDL cholesterol ratio, TyG index, and physical activity were all examined as risk or prognostic markers. Gut microbiome dysbiosis, maternal obesity, maternal protein restriction, aging, and metabolic syndrome were also implicated in CKD susceptibility or progression. Across these studies, chronic renal insufficiency was consistently framed as a global health burden affecting hundreds of millions of people and intersecting with diabetes, cardiovascular disease, and aging biology.
