Overview
Rheumatoid arthritis (RA) is a chronic systemic autoimmune disease characterized by persistent synovial inflammation, pannus formation, progressive cartilage and bone destruction, and frequent extra-articular involvement. It is commonly associated with autoantibodies such as rheumatoid factor (RF) and anticitrullinated protein antibodies (ACPA), which are linked to disease severity and, in some studies, cardiovascular risk. Clinically, RA causes pain, swelling, stiffness, functional impairment, and long-term disability, and it often requires prolonged treatment with conventional synthetic disease-modifying antirheumatic drugs (csDMARDs), biologic DMARDs, or targeted synthetic agents.
Pathobiologically, RA involves dysregulated innate and adaptive immune responses, including activated macrophages, fibroblast-like synoviocytes, T-cell subsets, cytokine networks such as TNF, IL-6, and IL-17A, and signaling pathways including NF-κB, JAK-STAT, NLRP3 inflammasome, and autophagy-related mechanisms. Because of this complexity, RA is both a major therapeutic target and a major model disease for studying immune modulation, precision drug delivery, and biomarker-guided treatment.

Recent publications portray RA as a highly active research target across immunology, pharmacology, nanomedicine, clinical outcomes, and health services research.
Several studies focused on the disease mechanism and cellular drivers of synovial pathology. One report described RA as a chronic autoimmune disorder with synovial inflammation, pannus formation, and progressive joint destruction, emphasizing precision delivery and modulation of the immune microenvironment. Another study highlighted aggressive synovial hyperplasia and joint destruction driven largely by apoptosis-resistant fibroblast-like synoviocytes (RA FLS), while a separate paper described RA as characterized by persistent synovitis and identified synovial fibroblast P2X7R as a potential therapeutic target in refractory disease. Additional work examined m6A-associated GAS6 expression in pathogenic activation of fibroblast-like synoviocytes, and another study linked insulin signaling to acquisition of an IL7R+ memory phenotype in PD1+ T cells in RA tissues, underscoring the role of tissue-resident immune cells.
macrophage biology was a recurring theme. Multiple studies described RA as involving polarized M1 macrophages that secrete pro-inflammatory cytokines such as TNF-α, IL-1, and IL-6 and activate JAK-STAT/NF-κB pathways. Related work investigated macrophage polarization imbalance in RA and tested interventions designed to restore immune balance, including cannabidiol combined with methotrexate, engineered nanomicelles co-delivering triptolide and calycosin, salvianic acid A delivery systems, and hybrid exosome-mediated delivery of sinomenine hydrochloride. Another study reported that tofacitinib repaired inflammation and mitochondrial dysregulation in GM-CSF-reprogrammed RA macrophages, reinforcing the centrality of macrophage reprogramming in disease control.
Autoantibodies and immune phenotyping also remained important. One study reported that RF and ACPA jointly influence the association between disease activity and cardiovascular risk in RA. Another pilot multiplex analysis used RA as one of several autoimmune groups to identify distinct immunological signatures for differential diagnosis of established and undifferentiated autoimmune diseases. A separate study examined PD1+ TIGIT+ CD4+ T cells as predictors of response to anti-TNF therapy in RA and spondyloarthritis, suggesting that T-cell phenotypes may help stratify treatment response. In addition, a study explored in silico design of a citrullinated protein-based multi-epitope vaccine, reflecting ongoing interest in antigen-specific approaches.
Therapeutic research was especially prominent. methotrexate was described as the anchor drug for RA, though its use is limited by hepatotoxicity, gastrointestinal intolerance, and incomplete efficacy in some patients. Glucocorticoids were characterized as fast-acting bridging therapy, and one cohort study examined their use during the first year of treatment. Comparative studies evaluated upadacitinib versus adalimumab over 7 years, sarilumab versus Janus kinase inhibitors in late- and younger-onset RA, and filgotinib versus tocilizumab in active RA. Real-world studies also assessed sarilumab effectiveness, safety, and patient-reported outcomes in Japan, and another study examined the impact of trial exclusion criteria on real-world comparative effectiveness of targeted therapies. A pharmacogenetic analysis reported that FTO rs7195994 is associated with TNF inhibitor response in lean RA patients, while another study assessed the adverse event profile of ocular injury associated with JAK inhibitors. A first-in-human trial of SOL-116 identified bile salt-stimulated lipase as a potential novel drug target in RA.
Traditional and adjunctive therapies were also investigated. Shaoyao Gancao Decoction was reported to ameliorate RA via inhibition of TNF-α/NF-κB signaling, and other traditional formulations or herbal products were studied for anti-arthritic effects in collagen-induced arthritis or adjuvant-induced arthritis models. Balneotherapy-related research found that low-temperature sulphurous peloids and sulphurous mineral water may help patients with active RA whose pain and functional limitations persist despite pharmacological control. Dietary intervention research suggested that Mediterranean-style eating may positively affect inflammatory indices in RA.
RA was also studied in relation to comorbidity, function, and outcomes. Physical function in older adults with RA was compared with population controls, and handgrip weakness was associated with motor cortex atrophy, suggesting that disability may reflect both peripheral and central mechanisms. Other studies examined obesity-related indices, geriatric nutritional risk, postpartum disease activity, renal complications, interstitial lung disease, cavitary rheumatoid nodules, IgA nephropathy associated with adalimumab, and susceptibility to nontuberculous mycobacterial infection under immunomodulatory therapy. Orthopedic outcomes were also assessed, including complications after reverse shoulder arthroplasty in patients with RA.
A substantial portion of the recent literature focused on advanced delivery systems and nanomedicine. Reviews and experimental studies described nano-drug delivery systems, synovial targeting, redox-triggered release, macrophage-targeted nanotheranostics, hyaluronic acid-functionalised solid lipid nanoparticles, mitochondria-targeted delivery, peptide-drug conjugates, and exosome-like nanovesicles. These approaches were often designed to improve delivery of agents such as chloroquine, triptolide, calycosin, salvianic acid A, naringin, kaempferol, and sinomenine hydrochloride to inflamed synovium or immune cells. Across these studies, RA served as a model for precision immunomodulation and targeted drug delivery.
Health systems and epidemiologic studies further showed that RA remains a major global burden. One analysis described RA as dominating the rheumatology AI literature, while another reported global inequities in biologic and targeted DMARD use. Additional work examined specialty pharmacy models, cost-effectiveness of vagus nerve-mediated neuroimmune modulation, and the burden of high-cost pharmacologic treatment. Together, these studies reinforce RA as a disease with substantial clinical, economic, and research importance.
